Tryouts for Baseball, Softball, Girls Soccer and Boys and Girls Track will be February 19-21. More sport specific information will be given to students and posted in the coming weeks.

All potential athletes trying out for a sport at Cleveland Middle must have a completed Final Forms Profile. Information on Final Forms is listed below. Anyone who comes to the first day of tryouts without having completed these forms may not be able to tryout until they are completed. This includes uploading your current sports physical to your profile. Please read the directions on how to set up your profile using the Final Forms Parent Playbook link. All other tryout eligibility prerequisites are listed below as well.

Eligibility Requirements for Middle School Sports:

-Must have passed 70 percent of classes (5 of 6) and have an overall 60 average from the previous semester.

-Must have been promoted from the previous grade.

-Must not be 15 before August 31, 2024.

-The student attended at least 85 percent of the total number of instructional days during the previous semester

-Must have a current sports physical signed by a doctor before the first day of tryouts.

-Must have fully completed all Final Forms.

We are very excited to announce that the CvMS Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms is free for all families and saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You will review your information once per school year to verify information is current and sign the annual forms. Click on the Parent Playbook (attached) to get started in FinalForms.
